Multidirectional flow analysis by cardiovascular magnetic resonance in aneurysm development following repair of aortic coarctation

چکیده
Aneurysm formation is a life-threatening complication after operative therapy in coarctation. The identification of patients at risk for the development of such secondary pathologies is of high interest and requires a detailed understanding of the link between vascular malformation and altered hemodynamics. The routine morphometric follow-up by magnetic resonance angiography is a well-established technique. However, the intrinsic sensitivity of magnetic resonance (MR) towards motion offers the possibility to additionally investigate hemodynamic consequences of morphological changes of the aorta. We demonstrate two cases of aneurysm formation 13 and 35 years after coarctation surgery based on a Waldhausen repair with a subclavian patch and a Vosschulte repair with a Dacron patch, respectively. Comprehensive flow visualization by cardiovascular MR (CMR) was performed using a flow-sensitive, 3-dimensional, and 3-directional time-resolved gradient echo sequence at 3T. Subsequent analysis included the calculation of a phase contrast MR angiography and color-coded streamline and particle trace 3D visualization. Additional quantitative evaluation provided regional physiological information on blood flow and derived vessel wall parameters such as wall shear stress and oscillatory shear index. The results highlight the individual 3D blood-flow patterns associated with the different vascular pathologies following repair of aortic coarctation. In addition to known factors predisposing for aneurysm formation after surgical repair of coarctation these findings indicate the importance of flow sensitive CMR to follow up hemodynamic changes with respect to the development of vascular disease.

Blood Flow Simulation in an Aorta with a mild coarctation Using Magnetic Resonance Angiography and Finite Volume Method
Coarctation of the aorta is one of the five main congenital cardiovascular failures, accounting for 6–8 percent of these failures. This research aimed to simulate the blood flow of a seventeen-year-old male teen with a mild coarctation at one-third of his aorta's descending branch. The simulation was performed by extracting the domain and the input pulsatile velocity signal as the boundary cond...
متن کاملFalse aortic aneurysm at site of previous coarctation repair: the role of cardiovascular magnetic resonance.
We describe a 37-year-old who presented with hemoptysis. Twenty-one years previously he had undergone Dacron patch aortoplasty for coarctation. Initial investigations failed to reveal the cause of the hemoptysis. Cardiovascular magnetic resonance (CMR) demonstrated an aneurysm at the site of the repair. He underwent successful repair of the aneurysm with a Gelseal interpositional graft.

A 69-year-old man was referred for investigation of a right upper chest mass. In 1965 he had an aortic coarctation resected with Dacron graft insertion. He remained asymptomatic for the intervening 33 years before presenting with breathlessness on exertion. A chest x-ray showed a right upper lobe mass, and computed tomography showed a descending aorta aneurysm. Cardiovascular magnetic resonance...
متن کاملMycotic pseudoaneurysm associated with aortic coarctation.
Development of a mycotic aneurysm or pseudoaneurysm after subacute bacterial endarteritis is uncommon. Nonetheless, patients with coarctation of the aorta are more likely to develop this complication. We describe a case of a large pseudoaneurysm discovered in a child with a previously undiagnosed aorta coarctation. Successful repair was performed with the aid of partial left heart bypass and th...
متن کاملImaging strategies of coarctation repairs
Background Coarctation is a common cardiovascular lesion accounting for 5 7% of all congenital heart disease. Without appropriate treatment complications are common and up to 90% of patients with uncorrected coarctation die by the age of 60. After repair close follow-up of patients is recommended. Recognised complications may include residual or recurrent coarctation and aneurysm formation. Fol...
